Housing Transitions began offering a Rapid Rehousing Program in October 2017, thanks to a grant from Housing and Urban Development (HUD) awarded to Centre County Government. Our Rapid Rehousing Case Manager accepts referrals and performs outreach to find eligible clients for the program. In addition, all residents of the Centre House shelter may be considered for Rapid Rehousing.
The goal of the project is to provide timely assistance to homeless individuals and families so they can quickly acquire permanent, sustainable, and independent housing. Clients secure their own lease. The program provides security deposit and first month’s rent, as well as a number of months of rental assistance, for people who are currently homeless in Centre County. Homelessness, in this case, is defined as individuals and families living in a place not meant for human habitation (including the streets or in their car), emergency shelter, or in a hotel or motel paid for by a government or charitable organization.
For agency referrals or self referral, please call our Rapid Rehousing Case Manager to discuss eligibility and receive a referral form.
