Who You Gonna Call? 2-1-1


Housing Transitions is the only 211 Coordinated Entry Access Site in Centre County. Coordinated Entry Specialists complete intakes, assessments, and send referrals to connect those experiencing housing crises to resources and other housing programs. In the last fiscal year, our in-house CE Specialist assisted 374 households in Centre County consisting of 680 individuals. Those individuals were connected to resources such as: emergency shelter, housing programs, homeless prevention and rental assistance programs, food banks, and other charitable organizations.

 

Our CE specialist works closely with other local organizations to connect with clients who otherwise may not have called to access services.  She is an integral part of our collaboration both in-house and within Centre County. She attends HT case manager meetings and county provider case conference meetings regularly. Clients have the option to call our Access Site directly or walk-in anytime during the hours of 9-4 Monday through Friday. Clients who are working or otherwise unable to call during the designated hours have the ability to schedule a time with our CE specialist that works with their schedules. 


On a recent Friday night a client was sleeping in their vehicle with no plan for suitable shelter, and our CE specialist worked tirelessly with a local organization until the client was able to check-in to an agency-paid hotel after 9 PM. This is an example of how our agency goes above and beyond to serve a population that sometimes gets overlooked. In addition to Centre County residents, our CE specialist can complete intakes and assessments for households located within the other 32 counties in the Eastern CoC.
